1. Initial Contact
Contact us to make an initial connection. If after contact you decide to pursue the full application process, completion of this first step will help us properly track your application from beginning to end.
2. Online Application Form
Find the online application form here. NOTE: This should be completed before submitting any of the supporting documentation below.
3. Recent picture of the prospective student

4. Standardized test scores

5. Most Recent Report Card/Teacher Progress Reports

6. ABA Treatment Plan
For students with ABA treatment plans in place, those can be uploaded here. Or, a paper copy can be mailed to The Grace School c/o Admissions Review TEAM 2418 East Joyce Blvd. Fayetteville AR. 72703
7. Neuropsychological Evaluation with Diagnosis
For students diagnosed with ASD, a Neuropsychological Evaluation with Diagnosis from a physician is required. The evaluation must be within the last 2 years. This can be uploaded here. 8. Behavior Support Plan, FBA
If the learner receives special education services and has a behavior support plan, this can be uploaded here. Or, a paper copy can be mailed to The Grace School c/o Admissions Review TEAM 2418 East Joyce Blvd. Fayetteville AR. 72703
9. Psychological or Educational Evaluation
If the learner has a psychological/educational evaluation, this can be uploaded here. Or, a paper copy can be mailed to The Grace School c/o Admissions Review TEAM 2418 East Joyce Blvd. Fayetteville AR. 72703
10. Speech Therapy Evaluation

11. Occupational Therapy Evaluation

12. Individual Education Plan (IEP) or 504 Plan
